If you are new client and would like to register with us or are contacting us on advise from your current practice for a second opinion please complete the below form.
Once submitted please can you contact your current or previous veterinary practice and ask they send your pets full clinical history to newclient@riversidevetsgrays.co.uk.
We will assess all submissions and get back to you within 2-3 working days once we have all the clinical history from your current or previous veterinary practice.
Completion of this form does not guarantee we will be able to register or assist you and your pet.
